Llergic reactions to -lactam drugs continue to be a frequent cause of adverse drug reactions. However, penicillin is no longer the most commonly used -lactam drug; patterns of reactions have changed as new chemical structures become available. Findings from a large data base of patients with allergy to penicillins are reported, including trends from the 1980s to the mid1990s. The analysis included 290 patients with immediate allergic reactions to a penicillin derivative. Patients with nonimmediate reactions were excluded, as were those with reactions to cephalosporins, monolactams, and other -lactams. Investigations included skin testing with major and minor determinants of benzylpenicillin BPO MDM ; , amoxicillin, and ampicillin and in vitro measurements of specific IgE. The clinical history was consistent with anaphylaxis in 71% of patients and urticaria or angioedema in 29%. Sixty-five percent of reactions were to amoxicillin. The next most frequent category, 15%, was "unrecalled penicillin"--ie, patients who reported penicillin allergy but were uncertain of the specific agent involved. Only 3% of reactions were to BPO. Seventy percent of patients had a positive skin test to at least one determinant, most often amoxicillin. Overall, 42% of patients tested positive to determinants generated from BPO, while 58% were "selective responders." Id. at 1352, 1355-56. Although the record showed that at the time of the amendment, only HPMC had been tested specifically with bupropion hydrochloride to achieve sustained release, HPC was known as a sustained release hydrogel-forming polymer in the art of pharmaceutical formulation, as evidenced by the Handbook of Pharmaceutical Excipients, other patents, and an Information Disclosure Statement submitted by Glaxo to the PTO. Id. at 1355-56. In support of its argument that magnesium oxide was a known stabilizer in the field of pharmaceutical formulation at the time of the amendment on November 11, 1987, Paddock cites to prior art references teaching the use of magnesium oxide as a stabilizer, the knowledge of the inventors of the `450 patent, and the testimony of experts. Use of a PPI, a doubledose histamine-2 receptor antagonist, or misoprostol along with an NSAID reduced the risk of peptic ulcer hospitalization by 39%. Abstract: Folate Vitamin B9, Folic acid, folinic acid, folacin, pteroyglutamic acid ; is essential for life-sustaining processes of DNA synthesis, replication, and repair which are naturally present in common foods such as peas, oranges, broccoli, and whole-wheat products. Folate levels have been associated with birth defects, cardiovascular disease, and many other important healthcare issues, which has resulted in government-mandated food fortification to deliver minimum levels of intake. Despite this one-size-fits-all recommendation by governmental regulatory bodies, studies suggest that a genetic predisposition may exist within as much as 67% combining both the CT and TT alleles ; of the population that causes a metabolic folate deficiency. Thus, genetic factors may play an important role in folate levels and metabolism. A substantial body of scientific evidence supports the importance of folate, genes associated with folate, genes associated with anti-folate therapeutics, and thereby a convergence in nutritional genetics or nutrigenetics. This review will comment on the substantial body of scientific evidence demonstrating the relevance for nutrigenetic measurements to guide dietary folate intake and nutritional supplementation with folic acid and micafungin.
